Application for Student Observation

Please answer all of the following questions to the best of your ability*, to help us better understand how your child performs as a student. This form is to be completed as a requirement prior to your child's scheduled observation.

*This form is accessible via mobile devices. However, desktop devices are best for optimal functionality. An inoperative “submit” button indicates that some form fields are missing or typed incorrectly. Desktop devices will describe any necessary corrections that hinder submissions, but mobile forms do not have this function.

      Observation Day Reminders Observations typically run for up to 45 minutes and, on rare occasions, we will suggest a longer time frame. Please send a snack and any comforting items necessary to help your child be successful and comfortable during observation. In order for staff to see how your child will truly function during the school day, you are unable to accompany your child during their observation. Please feel free to leave the campus to grab food, run errands, etc. However, we ask that you remain nearby close to the school in the event that the student observation ends earlier.

      Once you complete the Application for Student Observation, staff will contact you promptly within 48 hours for scheduling.

      Please understand that, due to increased interest and waitlists, failure to submit necessary documentation for your observation will result in immediate cancellation.

      Please notify us immediately if you will be late, if you must reschedule, or if you must cancel. After a 10-minute grace period, late arrivals will result in immediate cancellation of your observation. We will not be able to accommodate any additional parent schedule changes after two instances of cancellations, two instances of rescheduled tours, or any instances of no-call, no-shows.

      Immediately following your child’s observation, the observing administrator or teacher typically gives brief verbal feedback on their performance. Within two weeks, after a thorough document review including an analysis of Student Observation notes, the observing administrator will contact you to let you know whether your child’s enrollment has been accepted, denied, or deferred (pending open availability).

      A copy of your child's IEP is required by the date of your scheduled tour to help us better understand how they perform as a student. If your child does not have an IEP, a copy of their psychological evaluation will be required. Please upload your documents below.
