I understand the following PTO Policy:
1. A call out is unplanned time off. Three call outs result in a written warning. After two written warnings, or failure to provide documentation related to a call out, the employee will be placed on a final written warning or performance improvement plan. Consecutive days of call outs count as a single call out, however a doctor's note should be provided for any call out lasting 3 or more days. Doctor's note guidelines apply to absences related to caring for a child's illness.
2. A doctor's note must be provided upon your return for any absence on a designated blackout day. Failure to do so will result in an automatic write up.
3. Late arrivals of greater than 1 hour will result in a deduction from your PTO bank. After 3 occurrences of late arrivals without notice the employee will receive a written warning.
4. If there are extenuating circumstances in your life that impact your attendance, we greatly encourage you to consult with the Head of School so that reasonable accommodations can be considered for future absences.
